Wit Alive Youth Release of LiabilityI, the undersigned parent or guardian, hereby consent to my child (add name below), who is under 18 years of age, participating in the activities connect with the trip to and from and the time at WIT Alive in Tennessee, for an event sponsored by Whatever it Takes Ministries from August 31-September 3, 2018. I certify that my child is able to participate in these activities, including sports, swimming, etc. If my child has a medical condition which may be relevant to a physician in the event of an emergency, I have listen them below. In the event an emergency occurs, I may be reach at the telephone number listed below. If I cannot be reached within a reasonable period of time, I hereby authorize the adult sponsor, (add name below), to make emergency medical decisions for my child.
